The indications for intraocular lens removal can occur during intraocular lens implantation or late postoperatively. These indications can be classified into four major categories: intraocular lens damage, intraocular lens malposition, undesirable intraocular lens power, and undesirable intraocular lens performance. Removal techniques vary according to intraocular lens design, intraocular lens material, intraocular lens location, duration of implantation, and co-existing intraocular pathology.

Anterior Vitrectomy is a crucial tool in the skill set of the anterior segment surgeon. Although a planned anterior vitrectomy may be performed in such settings as traumatic cataract removal or secondary IOL placement, this procedure is most often an unplanned and unwelcomeaddition to a cataract surgery.

The Iris Claw Intraocular Lens (IOL) was originally developed for the treatment of aphakia in intracapsular cataract extraction surgery. Its indications have extended to include secondary IOL implantation to treat aphakia in cases with insufficient capsular support, including subluxated lens extraction, traumatized eyes, and keratoplasty.
